A rapid palatal expander (RPE) is a fixed orthodontic appliance that widens the upper jaw by separating the mid-palatal suture. It is most effective in children under 14-15 before the suture fully ossifies. The device is cemented to upper molars and has a central expansion screw activated daily (typically 0.25mm per turn, 1-2 turns daily). Expansion is usually completed in 2-4 weeks, followed by 3-6 months of stabilization with the appliance in place. A gap between the upper front teeth is expected during expansion and typically closes naturally. RPE corrects posterior crossbite, creates arch space for crowded teeth, improves nasal breathing by widening the nasal floor, and can reduce the need for extractions.
